As offenders near their release from prison, a Discharge Planner is assigned to help create a transition plan. The transition plan is essentially an addendum to the individualized program plan which is designed to ensure the offender will be receiving necessary services in the community post-release. A variety of discharge planning agencies provide services at RIDOC. Several agencies have a specific focus, for example, one agency works exclusively with offenders with severe mental illness, one with HIV+ offenders and one with offenders participating in residential treatment. The majority of all other cases are handled by staff from a local community action agency. In addition to pre-release planning, RIDOC is pleased to announce the creation of its new Transition Center, a one-stop resource center where released offenders can access community-based and state service providers, receive clarification about their Conditions of Probation (if applicable), and gain assistance with applications for public benefits.
